What is implantation failure?
Embryo implantation is a complex process that requires a healthy embryo and a receptive endometrium, as well as a close dialogue between the two. Implantation failure is diagnosed when it has not been possible to achieve a baby after at least three cycles of In Vitro Fertilization (IVF), or the transfer of more than ten good quality embryos.

Currently, there are highly effective solutions for implantation failure, among which the following stand out:

• Embryo transfer at blastocyst stage (day 5 of embryo development)
• Preimplantation genetic diagnosis
• Assisted hatching or assisted hatching

Causes

The causes of implantation failure are multiple and may be involved with embryonic problems or problems in the uterus.

Embryonic problems include:
•Chromosomal and genetic abnormalities
• Difficulty of the embryo to emerge from the enveloping layer (zona pellucida)


Problems with the uterus include:
• Anatomical alterations
• Inadequate endometrial development
• Benign tumours (myomas)
• Scars, adhesions or synechiae
• Infections or inflammation

Symptoms

Most women with pelvic adhesions have no symptoms, but some may have symptoms:
• Pelvic pain
• Intestinal obstruction
• Painful menstruation
• Pain during sexual intercourse
• Infertility
• Increased incidence of ectopic pregnancies

Can I have a baby if I have implantation failure?

Knowing the precise causes of implantation failure is critical when it comes to selecting a treatment that will achieve a baby with the least amount of wear and tear.

When implantation failure is associated with immunological problems, these can easily be treated with medication; whereas, if it is due to problems with the uterus, treatment varies according to the cause and can range from hormone therapy to surgery in very specific cases.
However, when implantation failure is due to embryonic problems, the main treatments accompanying In Vitro fertilization include:

• Embryo transfer five to six days after fertilization
Genetic Diagnosis for Aneuploidy (PGT-A) for embryo transfer without chromosomal abnormalities
Assisted hatching or assisted hatching to help the embryo break through the enveloping membrane
